This is the homespun wisdom Ken Coburn is imparting to the Secretary of Health and Human Services after learning Medicare is cutting funds for his program, <strong><a href=\"http:\/\/hqp.org\/\" target=\"_blank\" rel=\"nofollow\">Health Quality Partners<\/a><\/strong>, seemingly, because they can.\u00a0The program, based out of Doylesville, Pennsylvania, provides weekly nurse visits to Medicare patients who have at least one chronic disease and one\u00a0hospitalization within the past year. It&#8217;s a program that was approved by Medicare and extended, with great success, but now due to\u00a0confusing\u00a0bureaucratic lingo, the program is slated to be discontinued in June.<\/p>\n<p style=\"text-align: left;\"><a title=\"medicare\" href=\"https:\/\/www.dietsinreview.com\/diet_column\/05\/medicare-program-cut-for-scalability-despite-cost-saving-efficacy\/\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-88460\" alt=\"nurse resize\" src=\"https:\/\/www.dietsinreview.com\/diet_column\/wp-content\/uploads\/2013\/05\/nurse-resize.jpg\" width=\"600\" height=\"410\" srcset=\"https:\/\/dietsinreview.com\/diet_column\/wp-content\/uploads\/2013\/05\/nurse-resize.jpg 600w, https:\/\/dietsinreview.com\/diet_column\/wp-content\/uploads\/2013\/05\/nurse-resize-300x205.jpg 300w\" sizes=\"auto, (max-width: 600px) 100vw, 600px\" \/><\/a><\/p>\n<p>Patients with <strong><a title=\"obesity related diseases\" href=\"https:\/\/www.dietsinreview.com\/obesity-related-diseases\/\">chronic illnesses including heart disease<\/a><\/strong>, diabetes, cancer and even AIDS require long term care management.\u00a0There is often no cure for a chronic diagnosis, but with medication, lifestyle change and other interventions, people are living longer than\u00a0they did at the turn of the century. While some programs utilize crowded nurse call centers in different states for care management,\u00a0the Health Quality Partners model of getting nurses out in the field to visit patients was revolutionary and the results, undeniable.<\/p>\n<p>According to Mathematica Policy Research, the independent evaluator hired by <strong><a href=\"https:\/\/www.dietsinreview.com\/diet_column\/04\/medicare-offers-coverage-of-yoga-and-meditation-for-heart-disease-prevention\/\">Medicare<\/a><\/strong> to judge programs, Health Quality Partners\u00a0reduced hospitalizations by 33 percent and cut Medicare costs by 22 percent.<\/p>\n<p><!--more--><\/p>\n<p>With such impressive results, why is Medicare squashing funding for HQP? For starters, they list &#8220;scalability&#8221; as a contributing factor.\u00a0The HQP program is currently limited to Doylestown. Medicare is concerned that scaling up this small program to work nationwide, would not\u00a0deliver a big enough return.<\/p>\n<p>A frustrated Coburn has told <strong><a href=\"%20http:\/\/www.washingtonpost.com\/blogs\/wonkblog\/wp\/2013\/04\/28\/if-this-was-a-pill-youd-do-anything-to-get-it\/\" target=\"_blank\" rel=\"nofollow\">Washington Post<\/a><\/strong>, &#8220;People always ask if what we do is scalable. Well, define scalable. It\u2019s less\u00a0difficult than open-heart surgery, which is one of the most common surgeries in the country, and it\u2019s more difficult than giving a vaccine.&#8221;<\/p>\n<p>For <strong><a title=\"elderly death statistics\" href=\"https:\/\/www.dietsinreview.com\/diet_column\/06\/elderly-face-greater-risk-of-death-when-lonely-study-finds\/\">elderly patients<\/a><\/strong> enrolled in the program who don&#8217;t have close family (geographically or relational) having a nurse visit their home is a God-send. Due to\u00a0minimal time allotment for each patient, doctors often seem visibly rushed. Patients may leave without understanding a diagnosis or medication regimen because they feel guilty about asking too many questions.<\/p>\n<p>Bill Allen, a 79-year-old man enrolled in the program, says of his nurse, Patty Graefe,\u00a0\u201cBecause she\u2019s here in our home, you can feel more free to ask her anything. It\u2019s great.\u201d HQP nurses are doing more than taking vital signs, they&#8217;re building personal relationships.<\/p>\n<p>Coburn may be frustrated about Medicare cutting funds to the Health Quality Partners program but he&#8217;s undeterred saying, \u201cI really think we\u2019re just at base camp. There are so many contributors to health the system isn\u2019t addressing right now.
